Results may vary depending on individual health status, adherence to recommendations, and other factors. 3. Telehealth Consent - By using telehealth services, you acknowledge and agree to the following: Telehealth involves remote communication via video, phone, or secure messaging with a licensed healthcare provider. Telehealth is not suitable for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency facility. Providers will make recommendations based on the information you provide. Inaccurate, incomplete, or omitted information may affect outcomes. Telehealth services are provided in compliance with Minnesota law and applicable federal regulations, including HIPAA. You consent to the electronic transmission of personal health information and acknowledge the potential risks of electronic communication, including delays, interruptions, or unauthorized access. 4. Mobile and Remote Services - Mobile visits or services conducted outside of a clinical setting are offered at the discretion of Lumé Functional Health & Aesthetics. You acknowledge that mobile services may involve inherent risks, including environmental factors, equipment limitations, or delays in emergency care. You agree to follow all safety instructions provided by the provider during mobile or remote services. 5. Supplement & Product Guidance - Recommendations for supplements, nutraceuticals, or wellness products are provided by licensed healthcare providers based on the information you provide. Supplements and products are not intended to diagnose, treat, cure, or prevent any disease unless explicitly prescribed. You are responsible for reviewing ingredients, potential interactions, allergies, and consulting with your provider before use. By using our Services, you acknowledge that supplement efficacy varies, and Lumé Functional Health & Aesthetics makes no guarantees regarding results. 6. Acute Care & Aesthetic Services - Acute care services may include telehealth evaluation for mild or non-emergent illnesses, such as sinus infections, ear infections, or other minor conditions. Aesthetic services may be provided in-person or via telehealth consultation. Results can vary and are not guaranteed. You agree to follow all provider instructions for safe use of aesthetic treatments and acknowledge risks inherent in these procedures. 7. User Responsibilities - You agree to provide accurate, complete, and current information when using the Services. You will comply with all applicable laws, including restrictions on telehealth, supplement use, and aesthetic services in your jurisdiction. You are responsible for all decisions made based on your use of the Services and for monitoring your own health outcomes. You agree not to share login credentials or access for others without authorization. 8. Payment & Billing - Payment for Services is due at the time of service unless otherwise arranged. Fees will be disclosed prior to any service or consultation. All fees are non-refundable unless explicitly stated in writing. We reserve the right to modify fees at our discretion, with prior notice to affected clients. 9. Privacy & HIPAA Compliance - Lumé Functional Health & Aesthetics complies with HIPAA and other applicable privacy laws to protect your health information. By using our Services, you consent to electronic communications, storage of health information, and secure transmission of data as necessary for your care. 10.
